- Change of Major Form — Official form to be completed by student and submitted to Registrar. Signatures are required by current Academic Advisor, student, and Student Success coach
- Advising Agreement Form — Form to complete for approval by student’s academic advisor
- Liberal Arts Core Requirements — Source to compare student transcripts for assistance with selecting upcoming semester courses
